Updated Required Notice and Consent Forms

The following forms have been updated and uploaded onto the ISBE Required Notice and Consent Forms webpage.

• 34-57A: Parent/Guardian Notification of Decision Regarding a Request for an Evaluation

• 34-57B: Parent/Guardian Consent for Initial Evaluation

• 34-57B/C: Parent/Guardian Identification of Needed Assessments (for use with Forms B & C)

• 34-57C: Parent/Guardian Consent for a Reevaluation

• 34-57D: Parent/Guardian Notification of Conference

• 34-57F: Parent/Guardian Consent for Initial Provision of Special Education and Related Services

• 34-57i: Parent/Guardian and Student Notification of Transfer of Rights Due to Age of Majority

 A new form, 34-57P: Parent/Guardian Notification of Conference Identification of Needed Assessments, was created for districts to utilize when scheduling a meeting to determine the assessment tools and strategies needed to gather relevant functional, developmental, and academic information about the child, including information provided by the parent, that may assist in determining a child with a disability under 34 CFR §300.8.

 All notice and consent forms will be available in Arabic, Chinese (Traditional and Simplified), English, French, Gujarati, Korean, Polish, Russian, Spanish, Tagalog, Ukrainian, Urdu, Vietnamese, Ukrainian, Mongolian, and Hindi.
